HEREFirstly, you need to look at both your Terms and Conditions of Employment(T&C's), and your Company Sickness Policy to see if an extension is an option for anyone, or whether it could be discretionary.

HERE about the Equality Act... this has a few numbers on it to call , Namely ACAS, European Human Rights Commission (EHRC) and the Disability Law Service.
If you will have been on Company Sick Pay for six months (28 weeks) in August, you will have to claim Employment and Support Allowance. Your company should be sending you an SSP1 form to enable you to do this, and they should be sending this to you about now.
If , when your CSP stops , you have been off work for less than 28 weeks , then you will still get SSP for the remainder of that time , and could be entitled to Working Tax Credits if your family income is below certain thresholds.
Personally .. i would look at the T&C's of your employment and Sick Policy and then phone HR (or whoever deals with HR issues) and ask whether there is a possibility of an extension ( without mentioning Reasonable Adjustments or the Equality Act) .If August will be 28 weeks on Sick pay then ask for your SSP1 form.
